How Does a Rhinoplasty Work?

Surgeries that involve just a modest amount of reshaping are generally performed using a “closed” technique in which incisions are made only inside the nose.
If a more extensive revision is needed, the surgery may also involve an incision just below the tip of the nose, in the skin between the nostrils. Once the incisions have been made, we’ll reshape the bone and cartilage in the nose to accomplish your goals. These surgeries are outpatient treatments that involve anesthesia. Most surgeries last an hour or longer, depending on the extent of the revision.
Sometimes an Osteotomy May Be Involved
Some rhinoplasties involve an osteotomy, which is a controlled break of the nose using a tool capable of cutting bone with precision. An osteotomy may be incorporated if we need to realign bones to straighten a crooked nose. And if the patient is having a hump removed from the upper nose, a treatment that can leave the nose overly wide cosmetically, we may break the nose so that we can reposition it more narrowly.
Fortunately, osteotomies don’t increase a patient’s discomfort or recovery time.

What Is Recovery Like?
Most patients return to work and their regular schedules after about a week and resume exercise after two weeks. Initially, patients may have a small nasal cast or splint that protects the nose. It’s a good idea to have button-up shirts, rather than pullovers, on hand for the days immediately following surgery. Wearing button-up shirts minimizes the chance that the nose is jostled as it recovers.
Depending on the extent of your surgery, we also may recommend that you refrain from wearing glasses immediately afterward because of the way they pinch the upper part of the nose. If you regularly wear glasses, you may want to prepare in advance and have contact lenses, or another solution, ready.
When Will I See Results?
Although patients sometimes notice signs of improvement soon after their treatment, it usually takes several months for the full outcome of cosmetic nose surgery to be apparent. Will I Have Scars Afterward?
If your surgery is performed using the closed technique, where all incisions are kept on the inside of the nose, you won’t have to worry about scars. If we need to make an incision at the base of your nose between the nostrils to perform a more extensive revision, that tiny scar would be hidden beneath the nasal tip.
When Is the Right Time to Pursue Cosmetic Nose Surgery?
That answer is different for every patient. The right time to pursue treatment is generally when you have decided that you want to take steps to resolve a nose that has been bothering you. Patients between the ages of 20 and 40 make ideal candidates because the skin remains fairly elastic during those years and can easily adapt to the nose’s new shape. But we have successfully performed this treatment on many patients over the age of 40.
